To submit a New Patient Request, fill in all fields for the Patient.
If you are a guardian submitting a request for a minor, please submit their information. You may also put your (the guardian’s) information in the Any Other Information You Would Like Us To Know? box.
After submitting this request, it will be sent to our office. Upon receiving this email, we will enter you into our systems. We primarily use Therapy Appointment and/or Alma. Please be on the lookout for email notifications from them and complete the steps requested of you.
A member of our team will reach out to contact you. It may be the counselor who is best suited for your case who will contact you first, or it will be from contact@twistedmindscounselingcenter.com asking to verify insurance or any other information needed.
Our typical response time is 1-2 business days. If you have not heard back in that time, please send an email to contact@twistedmindscounselingcenter.com. We will quickly respond and help process the next steps to start the counseling sessions at Twisted Minds Counseling Center.
For General Questions, please fill out your basic contact point so we may reply back to you and the question you may have.
You may send a direct email, text, or call with the question you may have. Our response time on questions are 1-2 business days. If it is an urgent question, a text may be the fast solution to receiving an answer.
You may include a question in the bottom box (Any Other Information You Would Like Us To Know?) of the New Patient Request form. We will answer your question in a timely.
